Submit your completed application forms to Advanced Solutions for Kids with ALL supporting documents.

Documents Required:

  1. Childcare Application for Enrollment Form

  2. Advanced Solutions for Kids Photo Release Form

  3. Advanced Solutions for Kids Class Pet Permission Slip

  4. Authorization for Student Pickup

  5. Advanced Solutions for Kids Discipline Policy

  6. Copies of Immunization & Physical Health Records (for Toddlers & Pre-Kindergarten programs)
